Equine health plan membership terms and conditions
When joining the equine health plan you are accepting these terms and conditions. Please read them carefully, and ask a member of staff if you have any queries. A larger print copy is available at reception.
- The equine health plan is a rolling annual preventative healthcare programme. It is not an insurance policy.
- Membership constitutes an agreement between you and Hatston Veterinary Centre. Membership and benefits are not transferable to another practice.
- You will receive discounted products and services during your membership including, but not limited to, vaccines, dental treatments, worm egg counts and a full health check with one of our vets. Full details are available from the practice.
- Your horse can still receive treatment outside the scope of the plan, charged in accordance with the practice's normal fees, terms and conditions.
- These terms should be read with any additional literature provided by the practice detailing what is included in the plan; that literature forms part of these terms.
- You are responsible for following our vets' and nurses' guidance, and for making sure your horse is seen regularly for the preventative healthcare checks included in your membership. If we are unable to maintain your horse's health because guidance hasn't been followed, we may need to terminate your membership, in writing and with immediate effect.
- Membership fees are collected monthly by Direct Debit. We use Easy Direct Debits Limited to collect Direct Debits on our behalf, and your bank statement will show a payment to Easy Direct Debits on behalf of Flett and Carmichael. Your agreement is with Hatston Veterinary Centre T/A Flett and Carmichael; Easy Direct Debits Limited provide support to the practice, including transferring your payments.
- Membership for each horse renews automatically on the anniversary of the date your horse joined the plan.
- We will tell you in advance, in writing, of any change in membership fees on your renewal date, giving at least 30 days' notice of any change resulting from our annual review.
- Failed Direct Debit payments significantly increase administration costs. We reserve the right to charge an administration fee of £6 for each failed payment, added to your account.
- After a failed payment we will re-present our request to your bank after 3 to 5 working days. If the second request also fails, a second administration charge may be added, and a third and final request made after a further 3 to 5 working days. If that is unsuccessful, membership is cancelled automatically and your horse will no longer receive the associated benefits and discounts.
- If membership is cancelled automatically because of failed Direct Debits, your account will be reviewed and you will be charged the full price of any products and services received during membership, minus membership fees received to date.
- You may cancel your membership on your anniversary date by giving us not less than two weeks' notice. If you cancel before your anniversary date, we will review your account and, where applicable, charge retrospectively the full price of any products and services received, minus membership fees received to date. We may end our agreement by giving you written notice.
- Unpaid bills relating to membership fees, treatment or medicines dispensed will be handled under our standard terms and conditions (available on request) and may be referred to a third party debt collection agency.
- Notice must be given in writing, which includes email, post or hand delivery. We will use the most recent address you have given us. - Should you ever have cause to complain, please follow the practice's normal complaints procedure (available on request).
- Easy Direct Debits Limited and Flett and Carmichael will hold and use your personal data (as defined by UK data protection laws) to administer your preventative healthcare plan. Both may record and monitor telephone calls for training purposes and refer to them in relation to future queries. We take all reasonable precautions to keep your data secure and will not share it unless legally required to.
